Our Chronic Conditions Program

The Chronic Conditions Program funded by our PHN aligns with the National Strategic Framework for Chronic Conditions. The program funds prevention and management services to prevent the development of chronic conditions as well as direct service delivery and care coordination.


Preparing for the future

The number of people with chronic diseases and mental health conditions in our region continues to grow each year. Our PHN has limited resources and we work across the system to direct these to where they are most needed. Together we use feedback from our communities, our stakeholders and our data to determine if these needs sit with specific cohorts of people or specific regions. 


Our PHN is looking ahead to plan how we can enhance our impact on the health of our growing region. We look forward to working with our stakeholders to support people living with chronic conditions, and the health systems that cares for them.

My health for life

My health for life is a free behaviour change program designed for Queenslanders at high risk of developing a chronic condition and shows participants that making small lifestyle changes can have major health benefits. The program works in partnership with primary care and is a practical extension of the advice given by GPs and nurses to their patients. Participants can choose to complete the program via face-to-face, video conferencing or telephone coaching. The program’s eligibility criteria has been expanded in 2022 to further reach populations in need.


The program engages with allied health and general practice through:

  • quality improvement activities including identification of eligible patients through PenCS
  • program promotion and education during primary care education sessions
  • regular program updates through newsletters and emails
  • facilitator recruitment promotion through allied health newsletters
  • promotion of local programs in the region via website and health professional newsletters
  • regular website updates.

The Root Cause 

Supported by Darling Downs Health and proudly funded by  our PHN, The Root Cause's Children’s Health Program is an ongoing partnership with schools and families, designed to educate and empower children to make better food and sleep choices that will improve their behaviour and academic performance for the long term.



The Root Cause educates families on how their food and sleep habits impact on their overall wellbeing and ability to be their best. The initiative has educated and empowered over 43,000 children, parents, and teachers from over 175 schools across Australia. The program is developed to be fun, with results that prove that through the program, children make better choices around food and sleep.


In the 2021 school year, the program was delivered in 20 schools within the Darling Downs region and reached 4,996 students.
